Llamau’s mission is to end youth homelessness and homelessness for women in Wales. It’s a big ambition and to some people it might seem impossible, but we know that if we all work together, homelessness genuinely doesn’t have to exist.

An exciting opportunity for a Transition Worker in Swansea has become available.

This is a new service which aims to ensure that young people have a smooth transition into their accommodation and that they receive the right support to sustain their tenancy. 

The support and services delivered by the Transition Worker will be available prior to a move-in to, and move- on from supported accommodation, therefore supporting young people at two key junctures of their accommodation pathway, and when more intensive support and resources are needed.

Early intervention and prevention is a key aspect of the Transition Worker role, with mediation being a vital and significant service offered.  Using your support expertise, and by undertaking specific and focused housing transition work, you will work alongside Llamau colleagues, external partners and others to support young people to develop the skills they require.

You will be working alongside passionate and supportive colleagues in a rewarding environment, where you will be making a huge contribution to creating a Wales without homelessness, every single day.

Location: Swansea

Salary: £21,276 per annum

Hours: Working hours are 35 hours per week. Monday to Friday between 9:30am - 5pm. Please note that flexibility is required for this role to meet the needs of families is paramount, so some evening and weekend work may be required. Plus On Call rota

Contract: 1 x Full Time, Fixed Term - 12 moths from date employment commences
